Locate Your Insurance Information

We gladly welcome both insured and uninsured patients to our office. Additionally, we offer flexible payment plans through our partnership with CareCredit.

Before scheduling your appointment, please have your insurance information available. We will need the policy holder’s name, birth date, and Social Security number, as well as the insurance ID number (if applicable), group number, and insurance company. If you have an insurance card, we will guide you through how to submit it to our office.

Please note, due to the variety of insurance plans, we are unable to provide guaranteed estimates for services, as coverage may vary based on your specific needs and policy.

X-Rays/Treatment Info from Previous Dentist

If you have had any x-rays taken at another office within the past 3 years, please request that they email them to us at westenadmin@marqueedental.com.

Medical History

Your medical history is important to us because oral health is an integral part of your overall well-being. Let us know if you have any medical conditions, such as high blood pressure, diabetes, or artificial heart valves/joints, that may impact your dental treatment. Please be sure to complete your medical history (available on our forms page: https://forms.patientconnect365.com/15665) as thoroughly as possible.

If you have had a joint or heart valve replacement, you may need to take an antibiotic prior to dental treatment. Please contact the physician who performed your surgery to determine what is recommended. If you are aware of the required medication, we are happy to call in a prescription to your pharmacy.
